How to change name by Deed Poll in Cork?

Are you changing your name using the deed poll process?
Since being appointed as a Commissioner for Oaths in 2008 we have successfully signed and rubber stamped many deed poll change of name documents. The process has been made more user friendly since late 2024. The Courts service no longer require Cork residents to travel to Dublin, instead an applicant can post their documents, after they have been signed by a Commissioner for Oaths.

We can oversee you signing the
- Deed poll change of name form
We can then sign and rubber stamp the
- Affidavit of Attesting Witness – which is signed by a 3rd party – known as an “attesting witness”- this would typically be a friend of the person who is changing name. The witness must be independent, which is understood to mean over 18, not related, and living at different address.
We can then
- Certify that your a copy of your current Passport, Driving Licence, and Birth Certificate are true copies of the originals.
You then have the stamp duty affixed to your deed poll
