Changing a name using the deed poll process

Since being appointed as a Commissioner for Oaths in 2008, we have assisted with the signing and stamping of many Deed Poll change of name applications.
The process has become more user-friendly since late 2024. Cork-based applicants no longer need to travel to Dublin—documents can now be completed locally and submitted by post to the Courts Service.
We operate a mobile service across Cork City and suburbs, with a particular focus on evening appointments—ideal for those with busy schedules.

What Happens at the Appointment
Three people must be present.
- The person changing their name
- An attesting witness
- The Commissioner for Oaths
Attesting Witness Requirements:
- Must be 18+
- Not related by blood or marriage
- Lives at a different address
- Typically a friend or neighbour
Documents
1. Deed Poll Form
- Prepared in advance by the applicant using full names (downloaded from courts.ie)
- Signed by the applicant and attesting witness. The Commissioner for Oaths will see the deed poll, but does not sign it himself.
2. Affidavit of Attesting Witness
- Also prepared in advance by the applicant using full names ((downloaded from courts.ie)
- Signed and stamped (with his name and address stamps) by the Commissioner for Oaths
3. Certified copies
The Commissioner for Oaths will then certify true copies of:
- Birth Certificate
- Passport
- Proof of address (e.g. utility bill or bank statement)
- Any supporting documents (e.g. marriage certificate)
Typically, 4+ documents are signed and stamped in total. That concludes the involvement of the Commissioner for Oaths.
After the Appointment
- The name-changer goes to a Courthouse that has a franking machine to have stamp duty applied to the deed poll. At the time of writing that is €60.
- The full document bundle is sent by registered post to:
Courts Service, Four Courts, Dublin (see Courts.ie for full address) - The Courts Service returns the enrolled Deed Poll (officially stamped)
This enrolled document is the formal legal confirmation of the name change.
